Prescription Verification Policy
Requirements for submitting and verifying eyeglass prescriptions

Valid Prescription Required
All prescription eyeglasses orders require a current, valid prescription from a licensed optometrist or ophthalmologist. Prescriptions must be dated within the last 1–2 years, depending on your state's requirements.
How to Submit Your Prescription
• Upload a photo or scan of your written prescription during checkout
• Enter your prescription details manually, exactly as written by your doctor
• Provide your doctor's office name and phone number so we may verify the prescription directly, if needed
Pupillary Distance (PD)
An accurate pupillary distance (PD) measurement is required to properly fit your lenses. If your PD is not included on your prescription, you may provide it separately or request guidance from our team.
Verification Process
1. We review the prescription information submitted at checkout.
2. If needed, we may contact your doctor's office directly to confirm accuracy before your order is processed.
3. Orders may be placed on hold until verification is complete.
Customer Responsibility
• You are responsible for the accuracy of the prescription and measurements you provide
• Illusion Eyewear is not liable for issues resulting from incorrect or outdated prescription information supplied by the customer
• Expired prescriptions cannot be accepted; please renew with your eye doctor before ordering
